This case demonstrates that nontraumatic subperiosteal orbital hemorrhage may occur after thrombolytic therapy for myocardial infarction. Conservative treatment with cold compresses in the absence of visual impairment may be sufficient, as was in our patient and others.

Subperiosteal orbital hemorrhage in the postpartum period has been rarely reported. The authors herein present a female patient who developed acute-onset vertical diplopia, proptosis, mild retro-orbital pain, and restriction of upgaze immediately after labor. Neuroimaging revealed a subperiosteal hematoma along the right orbital roof.

Pathophysiology Orbital subperiosteal hemorrhage is caused by rupture of diploic veins between the periosteum (periorbita) and the bony orbit. Rupture causes a local hemorrhage with subsequent hematoma formation between the periorbita and bone. [eyewiki.aao.org] Subperiosteal orbital hemorrhage and compartmentalization of blood within the septal pockets (of the extra or intraconal space) may result in elevation of the orbital tissue pressure. This in turn may lead to decreased vascular perfusion and ischemia of the optic nerve or the retina with visual loss as the most ominous complication. At MR imaging, bone marrow is heterogeneous on T1- and T2-weighted MR images, and subperiosteal hemorrhage can be seen as periosteal elevation with increased subperiosteal T1 and T2 signal intensity (55–57). Figure 20. 2011-05-01 · Subperiosteal hemorrhage of the long bones occurs mainly in patients with various conditions attributed to abnormal collagen production, such as neurofibromatosis, scurvy, and copper deficiency, even though the preceding trauma is minimal [1–6].

The zone of provisional calcification extends beyond the margins of metaphysis resulting in periosteal elevation & marginal spur formation. Periosteum might be elevated due to subperiosteal hemorrhage (due to increased capillary fragility).
